Case Management

Case Management for either children or adults is a voluntary service to help individuals with a developmental disability or autism and their families to identify their needs, plan for their future and explore options available within the community.



Case Management includes:

  • Planning for immediate and future needs using a Person Centered Approach
  • Identification and referral to services and supports in the community
  • Support to navigate and coordinate services and supports
